Diagnostic finding

Elevated PTH-related peptide

Abnormally increased parathyroid hormone-related peptide above the reference range (usually undetectable or <2.0 pmol/L), resulting in PTH receptor-mediated hypercalcemia and suppressed endogenous PTH. Usually related to humoral hypercalcemia of malignancy (lung cancer, breast cancer, neck cancer, bladder cancer, ovarian cancer, lymphoma, myeloma).

An abnormally increased concentration of parathyroid hormone-related peptide (PTHrP) above the reference range (usually undetectable or <2.0 pmol/L), causing PTH receptor-mediated hypercalcemia with suppression of endogenous parathyroid hormone (PTH) secretion.

Causes

  • PTHrP-producing tumors --> Hypercalcemia of malignancy

    • Lung cancer

    • Breast cancer

    • Head and neck cancers

    • Bladder cancer

    • Ovarian cancer

    • Lymphoma

    • Multiple myeloma

  • Pregnancy-related PTHrP elevation

  • Lactation-related PTHrP elevation

Also known as: Elevated PTHrP, Increased parathyroid hormone-related peptide
