Elevated MCV

An increased mean corpuscular volume (MCV), indicating red blood cells larger than the normal reference range (approximately 80-100 fL) due to impaired red blood cell maturation, altered DNA synthesis, increased membrane production, or increased numbers of immature red blood cells (reticulocytes).

Causes

  • Nutritional deficiencies

    • Vitamin B12 deficiency

    • Folate (folic acid/vitamin B9) deficiency

  • Liver disease

  • Alcohol use

  • Hypothyroidism

  • Myelodysplastic syndromes

  • Reticulocytosis due to hemolysis or blood loss

  • Smoking

  • Pregnancy

  • Medications

    • Chemotherapy

    • Antiretroviral medications

    • Anti-seizure medications
